Understanding the four main rating categories and how applicants fall into each.

Insurance companies organize term pricing around rate class. Though terminology shifts between carriers, the structure stays constant: Preferred Plus (sometimes Super Preferred or Preferred Best), Preferred, Standard Plus, and Standard, plus additional tiers for elevated risk. A separate classification ladder applies to tobacco users with higher premiums.

What underwriters look at

Carriers evaluate body composition, blood work values, hereditary illness patterns (especially early heart or cancer), MVR findings, medication record, work and recreational pursuits, and nicotine exposure. Top ratings require excellent outcomes on essentially everything; Preferred permits minor variations; Standard Plus and Standard accommodate well-managed issues like controlled blood pressure.
